Prior to your first home visit, Emma will ask you to complete the Mercy Health Safe Home Visit Risk Assessment Tool as pictured below.

Please let Emma know if the answers to these questions change prior to any subsequent home visits.

Please have a pillow, 2 towels and a face washer ready for your appointment.
